What Is the UCC-1Ad Form?
 
The UCC-1Ad is a supplemental page used exclusively in conjunction with the primary UCC-1 financing statement. It is not a standalone document and is invalid if filed without an accompanying UCC-1.
 
The addendum provides dedicated, expanded sections to list additional information that exceeds the capacity of the initial form, ensuring compliance with state filing requirements under the UCC.
 
When to Use the UCC-1Ad Addendum
 
The UCC-1Ad form is typically required in specific scenarios to ensure the completeness and accuracy of the public filing:
 
1. Listing Additional Debtors or Secured Parties
 
The standard UCC-1 form provides space for only one debtor and one secured party. If a transaction involves co-borrowers, multiple lending institutions forming a syndicate, or a second secured party:
  • The names and addresses of all additional parties must be clearly listed in Items 10 and 11 of the UCC-1Ad.
 
2. Lengthy Debtor or Secured Party Information
 
If the legal name or address of the primary debtor or secured party is extensive and does not fit entirely within the allocated fields on the UCC-1 form, the continuation of that information is recorded on the UCC-1Ad.
 
3. Fixture Filings and Real Estate-Related Collateral
 
When the collateral involves assets that are or will become "fixtures" attached to real property (e.g., HVAC systems, machinery permanently affixed to a building), or collateral related to real estate like timber to be cut or mineral rights, the filing must meet specific real estate indexing requirements.
 
  • The UCC-1Ad is used in Items 13 through 17 to provide a description of the affected real estate, indicate the intention of a fixture filing, and list the name of the record owner if different from the debtor.
 
4. Debtors Who Are Trusts or Decedents' Estates
 
For specialized security interests where the debtor entity is a trust or a decedent's estate, the addendum provides specific checkboxes (Item 12) to correctly identify the nature of the debtor and provide the relevant trust file number or legatee's name.
